checked boxes on form disappear once the document get "ready" to be signed in Docuserve

I have a court form that has boxes that can be checked.  When I upload the form, the appropriate boxes are checked.  When I “view” it, the appropriate boxes are still checked.After I add a recipient and the form gets ready for signature, the color of the form changes and all boxes become unchecked.  The text boxes are fine, its just the boxes are now unchecked.Help!

Voting slip for shareholders

I need to send a voting slip out to about 70 individual shareholders to get their recorded approval (or not).They need to each sign and date their own version of the slip.I have the Business Pro subscription but the bulk send I was told would do the job for me seems to apply only to multiple signatures on the same document.We can’t do this. Every signatory has to be on a different doc.Can I do this without repeating the process 70 times ? If so how ?

I'm not seeing the full version of what i sent.

After I add items to my template and send to the client, I have set it up to send me a copy.  Problem is that only the client is getting the full version as I sent it and I am only getting it without the information I have added so I’m basically only able to view the template version of what I sent.  How do I change this?

I decided to pay a year in advance to use docu sign instead of e sign on our mls and still having trouble.  Don’t have any idea how to start and need a tutorial perhaps on it.Thanks.

Y/N Radio Buttons Open Conditional Field

I have a series of “Yes” “No” radio button groups. I need an explanation if any of the radio buttons are marked as “No.” It looks like I can’t have the same field be required if any of the answers are “No,” since it appears a field can only be conditioned on one action. How can I require this text field be completed if any of 7 radio button groups is answered “No”? Thanks! ETA: Basically, I need to require a text field if: Radio Group 1 = "No", OR Radio Group 2 = "No", OR ..... Radio Group 6 = "No". I can do the advanced conditioning in Web Forms, but it doesn't appear I can do it in a regular template.

Contract Problem

We signed a docusign contract with a boat yard in August of 2023. Now they are claiming they “lost the contract” and we need to sign a new one. Can someone please tell us how we access what we signed?We now believe they had all the copies of completed go to them, we cannot find where the completed document was emailed to us, but have found where we, as the signer, are registered under their email, and the document was voided. We have the number from where it was voided.

Envelopes

Hi, i had the basic real estate plan that only allowed 5 envelopes to be sent a month. Once i reached that quota, i had to upgrade to continue sending docs to be signed. I upgraded to NAR real estate plan which was like $20 more but i still cant send documents, keeps telling me to upgrade. Kind of confused how envelopes work on docusign. Which plan do I neded to keep sending more envelopes? Or can i somehow re-use envelopes, or does 1 envelope mean only 1 time sent? Sorry for the confusion but not sure what plan i need since it doesnt specify under plans

I want to download all Docusign documents  for the past 12. months. The drop down menu permits previously signed documents only in the past six months. Can you help?

identify payment element

Hello,In an envelope template I have a payment item to sign and pay at the same time.I need to be able to give the envelope template a Value  (which belongs to the record that generates the document to be signed) to be able to have traceability in my applicationCould someone tell me if it is possible? I have tried to enter a field as if it were a formula, but no, in the payment (in my case Stripe) it gives me the text, not the formula

Template with automation

Hello! I am having to issues trying to get my Template to do what I want.I have it set up that the customer has to click the YES Radio Button to unhide the Signature box.The radio button group wont Autoplace.There are 3 drawings per page and I can have 1 to 20+ pages.How do I get this to dynamically generate for each product on the page?

Extra envelopes

We almost reached our limit of 100 envelopes.We want to add an extra 100, but want to know if extra envelopes are only valid within the annual contract, or if leftover envelopes carry over to the new year.We'll not need all the 100 envelopes before the end of the first annual contract in October.  Thanks!

Remove or edit signing order for a CC recipient on a sent envelope.

Using Correct, I am unable to remove or reorder signing order of any CC’s on a sent envelope. Once the envelope is sent, the option to modify a CC recipient is greyed out, but all other recipient types are editable. The documents in the envelope have not been viewed or signed. Ultimate goal - I do NOT want the CC to receive any notifications of a completion, void or expiration. Please advise if this is possible.

Duplicate copies of completed PowerForms

We have Power Forms that go directly to our accounting team when completed.  They are gettin duplicate copies of these completed forms.  Is this just us or is anyone else having this issue?

one time electronic qualified signature

I need a qualified elekctrnic signature foor signing a document in France. It is realy once, that I need it, as, in Holland (my nationality) this is totally not required. I tried to get the qualified signature via FranceConnect+ but that is only for the french themselves. I need to sign a document to end a company inscription what was done by mistake.How do I do this without opening a paid account with monthly payments?

How to reset an email that has already been used

Hello,I was wondering if anyone knows how to reset an accounts email as I have used my work email to create an account but the email won’t let me in through the company portal because there was an option I selected which I needed to skip in order to get access. I would like to begin the set-up process again and want to know whether I will be able to use the same email?Thank you.

report column names look like you can click on them resort. the arrows change indicating change in the sort based on the column but it doesn't work. if i click on status the arrow changes but the data does not.
