Web Forms

Is it possible to have more than one person sign a form similar to how Power Forms work? I’m attempting to create an intake packet for incoming patients (pediatric mental health), and we currently use Power Forms, but those aren’t so compatible with cell phones. In trying to get Web Forms to work, we have several “Release of Information” forms that need to be signed, however each form is going to have different items to share with different “Release Recipients” depending on the entity of that recipient, and the guardian doesn’t know what needs to be shared. For example, if we are performing services for a child, then we need to share only bare minimum with the school if they are on an IEP (assessment and discharge). But if we are to share with their pediatrician, then we would share much more  (assessment, progress notes, medication, crisis support, discharge, etc.).  Our staff has specialized training in knowing what needs to be shared with each entity in order to check-mark the ap

how can i transfer the web form created by a user to another user

Hello,I created a standard and tried to create in Webform the recipient list moved over but non of the fields moved over, is there a way to fix this, or do I need to recreate everything?

Hi, I am new to docusign templates and webforms.I have created a docusign loan application template and want to convert it to a webform so that I can obtain the following outcome ieA loan applicant applies online and is asked a series of questions to prequalifiy them or disqualifiy them from a loan. If they don’t qualify there is no point in continuing the loan applicaiton. How do I create using webforms a conditional If then statement or rule so that a clause or new document is generated and displayed to the user that says “ We are sorry but your loan application has not been approved”  in the event that the user selects either a radio button and selects Yes/No or a checkbox in one or more fields? Is there a way to stop the loan applicant from continuing to complete further questions in the webform loan application once the loan has been denied? (I realise that it is not logical for someone to do so, but not all humans are logical)

Hello!We have built order forms based on different templates. If we change the original word/pdf-document for example want to add totally new question in it. Do we need to built web Form all over again?We can copy the template and replace the original document,  and the start the Create Order Form building from the start. This is how we do it now.Is it possible somehow to do this all in the Web Form-Edit Form and in the template actions there is in the web Form.  “Edit template fields” or in the “View template actions”.  To some point we have been able create ne field in the Web Form, but when trying to activate we get error API reference name.Thank you for your help if somebody understood my problem:)  

Also is it possibly to edit the email view-title for one powerform/template so that in the last email after all participants have signed there would the final email that would read something like, “Your oder is now approved”“Thank you for your order!” or “Your order is now approved” 

We have received this error code and are not able to send any envelopes at this time.  Any recommendations? 

Greetings! I am exporting form data for use in a spreadsheet. Unfortunately, some of the text responses are cut-off.  128 characters seems to be the limit.  Is there a character limit for exports? Is there a fix so I can get complete data? Steps: Reports tab > Documetn Data > Webforms >Selecting the form I want to view and then filtering for date range and run report Reports is visible on screenSelect Download a report

While signing a We form in Dev environment, testers are presented with this error “Unable to load CAPTCHA verification. Please refresh this page”. Can anyone please suggest, how this can be corrected. Thanks  

Recently started using Web Forms and after created the fields and went to view template connections I didn’t see the mapped fields. how can I fix that and what caused this issue? 

When web forms are saved in Agreements, they use the name of the web form completed. Is there a way to customize this for each unique entry? 

Hello, I created an extensive Webform for my internal team, but some of the text fields need to be changed to validate as Number and Date. When I tried to update them, I lost the connection to the webform fields and couldn't search them. Is there a way to fix this, or do I need to recreate everything? Thank you!Priscilla

I don’t seem to be able to set up a DocuSign Retrieve search to be able to pull Webforms. Am I missing something?

We have an intake packet and there are a few separate forms that ask for things like Name, DOB, Person Filling Out Form, etc. I’m struggling with the fact that one must have a recipient in order to create a WebForm from a PowerForm. The Legal Guardian will be the one filling out this data for their child, so the top of the form automatically creates a place for them to enter their name and email address (red) as that the main recipient for this intake packet. However, on the form itself, it also asks for the Legal Guardian name, because for legal reasons, we need the form to outline who is filling out this data (blue). It’s giving me an error because it’s named the same thing. Do I delete the blue from the WebForm? And if I do, what about all those locations on the forms in which we need the Legal Guardian name on the form itself? Will the name still appear on that form once it’s been completed?  

HELP!!! I am trying to locate a VERY important document that I signed from my landlord on your site. I don't know where to look on Docusign. It's a copy of my lease agreement. Can you please tell me where it is stored? I've looked EVERYWHERE and spent hours on your site trying to find it. I'd really appreciate it if you can help me. Thanks !

How do you create a cell phone field on the web forms

Hi I am new to DocuSign, learning about webform. We would like to have QR code pointing to wherefrom URL. The issue we anticipate is that every time we update the webform which is highly likely to happen, it will generate a new URL. This will become overhead to manage QR code which may have been distributed by the business as hard copy posters etc. Is there a way to force updated webform to point to the same existing URL?

Is it possible to have those other than the signer and sender of a web form receive a carbon copy of the completed form?I have a routing group in the template used to create the web form and that is supposed to receive a carbon copy of the document after completed. However, for some reason the completed document is only coming to me as the sender and to the signer.I see the option to have “Additional Recipients” as part of the Web Form, but it looks like this is really additional signers. I’m not seeing a way to add copy-only recipients here. Am I missing something?

I just created my first web form and I wanted to know if the signer has the ability to sign right on the form without opening up DocuSign. Thanks! 

We currently only use Powerforms for new hires to fill out a new hire packet, including the W-4. We then manually transcribe all of the information into our payroll platform.We would like to be able to export the data from the new hire packet to streamline this process. Does Web Forms allow for the Form W-4 to be signed, that we could download the employee data AND save a PDF of the document for our records?  Thanks,Susie    

The WebForm interface is far superior to having a signer navigate a tiny PDF on their phone.  So why hasn’t Docusign enabled a WebForm to be the primary content of an envelope, pre-filled with content by the sender, like we can today with a regular PDF or Word template?

While using web forms the user gets a summary of his data. In our case is the header and the button text of the summary in german.How can I change the texts on the summary page to English?

Hi,I have just upgrade dmy plan to be able to use webforms, i have created a webform from a template, howevr on my test run after filling in the details on the webform, it has not then populated that data into the template when it goes for signature, i hope that makes sense?!
