Hi, I wanted to create a Webform for a Supplier Onboarding Form. I wanted to create a template with both form (A and 😎 and within the webform, let the respondent select which form they will be filling up. I used a radio button at the start to let the respondent select and it should bring them to the appropriate page with the relevant questions for the chosen form.

I tried to do a test run first and realized that once I filled up the form and send it off for signature, it shows both the pdf forms as one pdf. I was wondering if it is possible to set it so if the respondent selects form A then at the sign off it only shows form A with the filled up answer and vice versa instead of showing both form A and B but only form A being filled up. Or would I have to actually create 2 separate templates and webform for each form.

Hello @vanrocks2000,

Thank you for reaching out to the Docusign Community.

Have you tried to Add Conditional Rules to a Web Form? Conditional rules can be added to a web form to determine which fields are displayed based on recipient responses. These rules allow you to show or hide fields, pages, or sections depending on certain conditions. You can create rules using the "if ... then" paradigm, specifying conditions and actions based on recipient responses.

Hi Christopher,

Thank you for the answer! I did put some conditional formatting for my webform. But what I was thinking more of was on my template having 2 separate documents, and depending on the option chosen in the webform, it will display either the first or second document in the signing screen and hide the other. Not too sure if this an actual feature that is available though.

Hello @vanrocks2000,

Thank you for reaching back, have you tried to create a Conditional Fields within the template? Conditional fields allow you to create dynamic documents that show specific fields to recipients based on certain conditions being met, such as selecting a checkbox or dropdown option. These fields can be used to gather additional information or prompt the recipient to answer specific questions. Multiple rules can be created for a trigger field, allowing for different scenarios to be specified based on the recipient's selection.
