How to Change a field label name?

To confirm, do you want the field to be like the screenshot below?

If that is the case, you will need to add that to the text section of the field field properties, please see the image below:

And then save it as a custom field so you do not have to repeat the process each time to need to use the field. Custom Fields

Please be aware that during the signing session, the signer will need to delete the text in the field to enter the required information.
