I have built a webform for an application process that we have.
Some of the options are checkboxes and I have having difficultly linking the checkbox option in the webform so that it populates the relevant checkbox on the template.
These are the checkboxes in the webform (top) and the fields in the template (bottom).
I have tried all options and I cannot make this work - any suggestions on what I am doing wrong?

To help you better, I need to know what type of WebForms you are using, if it’s the "Use eSignature template" or the "Start from scratch" (used in IAM), and whether you are using Maestro WF (IAM product) to create your workflow or not. This information is important, as the order of creation is different for each scenario.
In any case, my main tip is to pay close attention to ensure that the names of the DataLabels in the WebForms and in the Template are exactly the same, as this is how Docusign can link the WebForm response to the Template.
