Is there a way to include a pop-up form appear within the envelope to have the customer complete some questions but also still completed the document signature?

For example, I would like to send a subscription renewal document (standard format for doc signatures) but also include questions like:

  1. Please provide purchase order number if available
  2. When is the expected date for budget approval?

The goal is to make this exchange more interactive.

Thank you Nathaly. This is a great start to get me thinking. Could these conditional fields support "calendar date" entries or is it just alphanumeric text entries only?

Yes, it does. If you need the recipient to enter the date, you can set a normal text field with a date validation or if it needs to be automatically populated, you can add a Date Signed field (either of them works with conditional fields).
