I received this response from DocuSign. I did have to Google have to convert a PDF from active to static. I followed those directions through Adobe and the documents uploaded correctly. Hope this helps you.

PDF is an Active XFA Form:

If your PDF is an Active XFA form, DocuSign may not fully support it, which can lead to form field data loss.

Solution: Use the application that created the PDF to save it as a Static XFA instead of Active.

PDF Flattening Issue:

If you are flattening the PDF, it can also result in the loss of field values.

Solution: Flatten the PDF by printing it to a new document using Adobe Acrobat Professional or Foxit Reader:

Open the file in Adobe Acrobat or Foxit Reader.

Go to File > Print.

Select Adobe PDF or Foxit PDF as the printer.

Choose the option to Shrink to Printable Area and click OK.
After applying these solutions, your form field data should be retained correctly.
