I am very new at this and have a generic template "terms of use" for my website. How do I create esignature without knowing the recipient name or email address?

I understand you would like to use your template in a way where the recipient is unknown.

With PowerForms, your business can generate on-demand, self-service documents for signature. You’ll be able to eliminate document preparation time (by using a pre-existing template), and easily pull collected data into your existing applications. A link can be generated that can be emailed, sent via SMS (on certain enterprise plans), or embedded into your website.

If you don’t have permission to create PowerForms, you should contact the DocuSign Admin at your company to see if it’s available.

I would suggest the use of Powerforms if you have that feature available to your DocuSign Account. Basically its an online Template and in the Template you setup a Recipient with only a Role such as "Applicant", when they go to your website they can click a link which is provided in the Powerform to then direct them to this online Template where they enter their Name and Email address and then can sign a 'Terms of Service' document.
