I have setup a paid Business Standard account on DocuSign and cannot obtain a W-9 from DocuSign to provide to by accounting department.  Several employees on the phone said that they would email it to me once we got off the phone and I never received an email.  One employee told me that once the paid account is setup, then I will be able to open a case and request one but my paid account is now telling me that I do not have support enabled to open a case.  No phone number or functional email. This company is a joke.  My company will be cancelling the credit card charge if a W-9 is not obtained ASAP.

I apologize for the inconvenience; Are you referring to a blank template you can use? Have you reviewed our Docusign Template Library? The Docusign Template Library is a resource for creating templates, and there is a video tutorial available to learn how to use the template samples. Or are you referring to a form filled out by Docusign?

Can you provide your email and account number via DM?
