To gain a better understanding of the situation, I have questions:

• Are you using the regular DocuSign Website (www.docusign.com) or an Integration to generate/send documents like; Rooms, CLM, SpringCM Salesforce, Workday, etc?
• Can you please describe, step-by-step, process of what you did? For instance, what field types did you add to your template, and is the field no longer there when adding and saving it? 
• Do you have the exact error or screenshots of the issue that you can provide? (please conceal any Personally Identifiable Information on the screenshots, as this is a public forum)
