Thank you for reaching out here in the DocuSign Community.

I apologize for the inconvenience, I understand that your signer did not fill certain parts of your envelope.

If the envelope is completed and if your account allows it you can use the forward option to have the recipients fill in the missing parts, Please see Forward a Completed Envelope

Or you can use the regular method and upload the document and add the fields that are needed.

But if you need to have all the information in a single certificate of completion a new envelope must be sent.

Note: Some advanced features and options are supported only in specific DocuSign plans. Your account plan might not support some options discussed in this help topic. Contact your account administrator for more information about which options are available for your account. I was also digging around on Google and found where to add a check box - which is ultimately what I needed.
