The 1st one is from the production environment, and the 2nd one is from the demo environment. I can understand the different aid, but why is it not matching the rest of the URL?

It should be the same except for the aid and domain URL.

Unfortunately, I can't answer your question with only this information. There are many other criteria that must be evaluated, both within the CLM environment and within Salesforce itself.

To me, the second URL looks like it’s cutting off the Folder Name and Folder Path that is defined as part of the Object Mapping in Salesforce. There should not be a difference between these two parts of the URLs just because one is coming from prod and one from dev. I would check the Object mapping in your Salesforce account from the Docusign Apps Launcher section as this is the part that controls this portion of the URL.

Is the process working when you click the button in Salesforce? What I think will happen is the doc gen form might open and the document might get generated *but* it won’t know where to save the document once you click save. If everything seems to be set up correctly, and the URL has been copied correctly from CLM, i would submit a support ticket like ​@Vinicius.Rodrigues suggested above!
