Check box in Workday

I am trying to create a check box in a document, utilizing docusign for e-sign. I have this set up in docusign: \YesCheckbox{r}\

and I have this on my document \YesCheckbox1\ 

when document is launched in Workday, no check box comes up. Signature/print name/date work just fine with the anchor text. 

What set up might I be missing?
