Web forms

Question

I am trying to develop a web form that allows someone to choose to change or cancel and if cancel, then given the option to start something else.  For example, we have retirement contributions that we would like the employee to be able to cancel current contribution and start another type of contribution on the same form without having to complete the form twice.
