I understand that a checkbox, previously visible in your template, is no longer appearing, and you haven't made any recent changes. We apologize for any inconvenience caused, and let's work together to sort this out.
A potential cause for this issue, especially when a template isn't password-protected, is that another user may have edited the template. Without password protection, other users can modify the template, potentially deleting fields like your checkbox. They might have, this is just an example, uploaded a new document, which would remove all fields, and then saved the changes.
I suggest reviewing the template history to identify any recent edits and the user who made them. This will help determine if another user modified the template. Template History
If you have a backup of the template, restore it to recover the missing checkbox. If a backup isn't available, you'll need to recreate the checkbox and save the template.
To prevent similar issues in the future, consider password-protecting the template or restricting template field modifications. More information on the articles below:

Thank you for responding, the template is not password protected and no one edited them. Further checking, i see it stopped working due to the conditional validation. When i removed it it just displays. This template was build and has been in-use for ~3 months without any issues but encountered this issue yesterday.
I had this checkbox to be displayed on conditional selection and it has been working until yesterday. Please find the error screenshot referenced below how it looks kind of whitespace not giving a cross as it gave be earlier (checkbox screenshot)

Thank you for getting back to me. We may need to obtain a copy of the template for review, so the best course of action is to Open a case in the Docusign Support Center. I would suggest downloading the template and attaching it when you submit the support case.
Alternatively, you can locate a previous envelope (from the Sent folder) where the template functioned correctly. From that envelope, save it as a new template and conduct testing. For more information, check this article: Save as Template
