Embedded Signing for Salesforce (issues with attachments and etc.)

Hi, I have issue with attachments, they just don’t return to Salesforce, my expectation is that after user signed doc, it should attaches to Salesforce record, I also adjusted Salesforce connection in Admin panel at DocuSign Demo and installed and connected package in Salesforce sandbox. I attached Apex file, could someone help me?
