Why radio buttons, like adding addendums, are not active?

It’s understandable how this unexpected issue could slow down your workflow and create delays.
I’ve reviewed the form and found the radio buttons and checkboxes to be functioning properly. Could you let us know if you access these forms through Workspaces or Rooms?
