Eliminate spurious envelope name prompts

I suggest two changes.  The first is to change the order of the fields on the envelope setup page so that selecting the SMS option occurs BEFORE a name is entered.  Then, when the name is entered, the correct mobile phone number will appear in the Phone Number field.

The second is to add a check box that says “Only include names from the contact database.”  This will eliminate spurious names prompts, which are a nuisance, for authors that are trying to maintain Part 11 compliance by only using names that have mobile phone numbers.
