Wrong Email & Name stamped on Envelopee

I have done custom from Salesforce to Docusign using Esignatue API and created multiple templates in Docusign now whenever an end user choose any template from Salesforce to send for E-signature, It show my “Name ” & my “Email Id” stamped on the email when the recipient gets it but why?, could anybody help here?
