As you mentioned, all your recipients have dedicated fields, but if you’ve only assigned signature or initial fields to some recipients, it may not be sufficient. 

The error is specific to having at least one of these fields: text, checkbox, dropdown, or radio field. Please double-check the template and ensure that each recipient has at least one of these fields.

If you confirmed that the following recipient already has one of these fields and is still getting an error, I would recommend that you open a support case, so that our support team can check this further. Open a case in the Docusign Support Center
