How to tally Checkboxes

I have a form where I have an evaluation attached, the user is required to answer a series of questions by choose 1 of 4 options per question and then tally up the total number of checkboxes chosen. Is this possible in a template? I know that there is a formula option but that seems to only be for numbers and dates that are being pulled into my template through a salesforce integration. The evaluations are solely in docusign and handed off to a provider after the client has gone through all of the consent forms.

To better assist you could you provide the current version of the DocuSign for Salesforce that you are using?
