Employee's Withholding Certificate: W-4 - Unable to create the envelope.

Hello there,

I’m trying to create a new envelope using a starter template:

Employee's Withholding Certificate: W-4

 

Keep getting the following error:

Unable to create the envelope.

 

it looks like all other starter templates work well, what am I missing? Tried both Chrome and Edge.
