We have approximately 350 users in our corporate account that utilizes the Part 11 module.  Out of those, I have one single user for whom the Electronic Record and Signature Disclosure appears in the Certificate of Completion.  The user himself does not see anything when he signs an envelope, but when completed, this is always included in the summary.  Does anyone know why this is happening, and if so, what needs to be done by myself as the admin, or by the user himself to remedy?

Can you provide a screenshot of the part of the certificate of completion you refer to? Is this the first time that you sent an envelope to this signer?

Thanks for responding!  Below is a screenshot of the top portion of the 3 page document that gets included when this individuals signs an envelope.  This person has signed many envelopes throughout the past several years and this apparently is produced every time.  It was just last week when somebody finally brought it to my attention.  I have not personally sent this individual an envelope.  Some additional background, as is noted in the screenshot, our DocuSign account is licensed through SHI.

Hi we have the same issue in my company. We always get the legal disclosure with the certificate of completion (for all users, for all envelopes and for normal and part 11 docusign). But our settings only require acceptance of the eres document at first login. Could you please help to clarify how to get the certificate of completion without the legal disclosure?

Thank you for reaching back.

According to the guide Certificate of Completion, if an envelope recipient accepts the sending account's legal disclosure, it is appended to the CoC, and the date of acceptance is listed. Depending on the account's legal disclosure settings, this date can differ from the signing date. For example, the account setting requires acceptance of the disclosure only for the first envelope signed from the account and the recipient previously received an envelope and completed the disclosure requirement. If the legal disclosure was not required due to the account settings or if a signer is a member of the sender's account, it is listed as "Not Offered via DocuSign" and is not attached to the CoC.
