Consent form to be signed by parent and child

I have a consent form that needs to be signed by teens (our participants) and parents prior to participating in a research study. I want to be able to email/text the form to the teens, they would then forward the link to their parents to sign, and the teen can also sign on a designated line. I do NOT have the parents’ names, I only have the teens’ names (that is our recruitment strategy). 

Is this possible? Ideally we would let the parent fill in their name and sign it. I can technically fill-in the teens’ names but that could be cumbersome at scale, so ideally they would also fill in their own name and sign. 

I am completely new to Docusign so would appreciate step-by-step instructions for this.

The best would be to set up a PowerForm or a Web Form to gather the name and email addresses of the parents, when the child fills out the participation form. In this case you only need to send out the link to the children to start the process.
