Checkboxes and Radion Buttons are unchecked after submission

I found an issue; after submit a document, any of checkboxes and radio buttons become unchecked, although I checked them before submit it. I wonder if you would expect possible causes.

More details;

  • Those checkboxes and radio buttons are required fields. They cannot leave those fields empty when submit the document.
  • The text line fields and signature are not erased. Those fields’ inputs are preserved without any problem.
  • This issue happens only one of my documents. Recently I replaced its source document, but the checkboxes and radio buttons are kept from previous version.

I’m sorry you are having an issue with the checkboxes and radio buttons being unchecked after sending the documents. We apologize for the inconvenience this has caused you.

I have checked the sample document attached and replicated the issue on my end. I uploaded the document to see if the same behavior occurs. I have not encountered any issues with the document, and the checkboxes are working as expected. Please refer to the image below for reference.

 

Also, I noticed that the checkboxes are optional fields, and there are no radio buttons in the sample document.

 

You can use the radio button fields to select from the yes or no option. Kindly take a look at the sample radio button field.

 

Please let me know if you have encountered any issues after reuploading the document to test. If necessary, provide a screenshot of how you were able to upload and any errors or pop-ups encountered during the process.

Thank you for responding.

I logged in to DocuSign and double-checked my template, and confirmed that those buttons are surely radio buttons and required.

However, I found that those fields are from the resource PDF. Recently I replaced that PDF with its new version, then a dialog of "Send a PDF with Form Fields" appeared. I supposed that choosing "Delete" would erase all existing DocuSign fields as well, but perhaps would not?

I appreciate your response and update.

When you select “Delete,” it will remove all the fields. Instead, select “Assign to”. This retains the fields and you may also edit if necessary.

Thank you again.

I mean, I would like to keep DocuSign fields but delete only the fields in resource PDF. Because the DocuSign fields are used in its old version, so I intend to use those fields rather than the PDF’s fields. Can I do that?

You can remove the PDF form fields from the resource PDF while keeping the existing Docusign fields, provided those Docusign fields were added as Docusign tabs and are not mapped to the PDF form fields.

If the Docusign fields were automatically converted from or linked to the resource PDF form fields during upload, removing the PDF form fields may impact the behavior.
