How do I create a form where users can select either "Option A" (checkboxes 1-4) or "Option B" (checkbox 5), but not both?

Hello @dsmith1

You can add the checkbox field from and the click on the “Plus” icon underneath to add more check boxes to the group.

Then in field property in the right hand side, you can put the validation of selecting any one by choosing “Select Exactly” .
