Bulk send CSV - Error 'Edit or replace the CSV to add values for the required Document Generation data fields.'

I prepared a template, it has data fields. Because the template includes two files, each of which has an ‘effective date’, the CSV includes duplicate columns. Dunno if that’s why I’m getting the error. I have verified my test file includes all values:

 

But I get this error:

It’s possible that the duplicate “effective date” columns are causing the error. Since your template contains two documents with similar fields, Docusign may be having trouble matching the CSV values to the correct Document Generation fields.
A good thing to check is whether both “effective date” fields use the same data label/field name. If they do, try giving each one a unique name, for example:
• EffectiveDate_Doc1
• EffectiveDate_Doc2

Then make sure the CSV column headers match those field names exactly.
You may also want to download the sample CSV from the Bulk Send page and compare it with your file to confirm the formatting matches what DocuSign expects.
Another quick test would be to temporarily remove one of the duplicate date fields and see if the CSV uploads successfully afterward.
