This looks like a good use case for a PowerForm.

A DocuSign PowerForm is a feature that allows organizations to create self-service documents that can be filled out and signed by recipients without the need for direct sender involvement. These forms are typically used when a standard document needs to be signed frequently by different recipients, such as in applications, consent forms, or surveys. PowerForms can be accessed via a unique URL or embedded directly into a website, making it easy for users to complete the required information and signatures on their own.
