Regardless of whether you want 1 or many checkboxes, it is added as a Checkbox Group. By default, it starts with 1 checkbox only which is what you desired in your question. You have to click the + under the checkbox to add more if needed.
In the properties on the right side, you can control the validation. The default is “Select at least 0” which means the signer has the option of whether or not to check the checkbox. If you want the checkbox to be required, change this to “Select at least 1”
