Document Generation - Format Variations

I have loaded a version of my document generation for NDAs but when it gets generated in the system, the formats change.  Can someone help me figure out if there is something needed on the formatting end to get it to show?  I have removed and replaced multiple times and when I see the document in my CLM folder it is perfect and showing two columns, but when I fill out the form fields and generate a document it removes the two column and changes it to a single column.

There is also a conditional logic I added but I made sure that there were spaces between the logic and the start of each text lines.  Any ideas?
