Conditional Logic for Signature based on # of Checkboxes Selected

I am not sure if this is even possible, but I am looking for help on requiring a signature if one or more checkbox is selected for this vaccination declination form. 

If one is selected in the above sheet, it will automatically select it here, but I can’t seem to attach the signature to the option of one or more? I even used validation, but not sure if I missed a step to get it to work? It works for one checkbox but not if we select the other 2 and not the one that it’s technically set up for. Any one have any ideas on this?

Have you tried to use a Conditional rule with the fields?

To define a conditional field in an envelope, start by adding files, recipients, and messages. Then, in the Add Fields view, add all the fields to your documents, including the ones that will be conditionally displayed. Select the trigger field on your document and define the logic for your rule in the properties panel. Finally, select the fields you want to show to the recipient when they trigger the rule, and your conditional fields are defined. Recipients will only see the conditional fields if they complete the trigger field according to the rule logic. Define a Conditional Field

Yes, this is possible, but requires a little workaround.

It looks like you have already discovered the conditionality in Docusign eSignature, which is great.

What you need to do is the following:

  1. Add an additional conditional text field to each of the checkboxes, make it read-only with the value 1, white font so it is invisible on the document. You could name them Check1, Check2, Check3.
  2. Add a formula field that sums up Check1, Check2, Check3 inside an If clause, to return 1 if the sum is equal or greater than 1.
  3. Add the conditionality for the signature field based on the formula field value being 1.
