We are sending out documents that need to either have yes/no checked, is there a way to integrate this? When using the checkbox option it does not state "required" I've also tried using the "approve" icon but when returned signed it does not display.

Checkbox fields are AND operators and do not have a Required option. Use the "Radio Buttons" field instead which work as OR operators and also can be set to Required. So for a YES and NO , you drag over the Radio button field, use the + to add more as needed, then adjust placement and set Required etc from the right control panel.
