How to create a form and generate a link to post on our website.

1. I had a lawyer create a legal binding petition that I need to gather citizen signatures. I uploaded the document, but I need to know how to add multiple fields and then post the link on our website. 2. These will be random citizens, so I do not have any contact info (email) to input on the "send" feature. any advice is welcomed. TIA
