You can print the form from Excel to PDF and upload it as a document on your envelope in DocuSign.   You might need to add additional tags to collect information from your employee.   You can also upload the Excel directly to DocuSign.   Please note, in either situation the document will be sent and received in the PDF format.

thanks for the quick reply.

To be clear I’m looking for a way to create a form with employee information in Excel, then send it to the employee as a PDF via DocuSign to fill in their information and then get a copy back to forward to our medical plan.

In Excel I have rate of pay, start date and so on. I will need to sign the document also.

Would you consider using the eSign API to create an integrated process between your Excel and DocuSign?  

Or would you manually create the form from Excel with all of the information and upload it to DocuSign. After it is signed, it would be sent to the medical plan?

Whichever is the most efficient use. I’d be fine with an API add in for Excel so that I could fill in all the information in Excel. Write the VBA code to send to sign and then have the employee fill in their part

The API route would be the most efficient, if you want to put in the effort to set everything up and have the skillset inhouse to do it.
