I received a Docu-sign from City of Hope. When I went to sign, my husbands signature kept popping in the signature line. I could not get my signature on the Release Form. Eventually it signed his name and sent it to City of Hope. What can I do?

Sounds like the envelope was sent to a Recipient, bearing the specific email address and specific name of your spouse. So the signature is basically linked to an existing DocuSign Account. I would have suggested refusing to sign (void) or possible commenting back through the envelopes that they should change the email address to your email address. At this point if you signed there is not much to do except contact the Sender and determine next best actions. Still possible they can void, correct, or just send a new envelope.
