This is my first time using DocuSign. I have sent a release form to 15 recipients. I have received it so far from 2 but when I look at the document, both signatures and name fields are overlapped meaning I can see both signatures and names on top of one another. I've obviously screwed up something when setting up the form but not sure what I've done. Has anyone else had this experience or anyone able to assist?

It sounds like you may have sent one envelope with 15 signers in it, but you may have wanted to send 15 envelopes with one signer in each. If you really want to have 15 independent envelopes, you could consider creating a template and sending an envelope to each signer with that template. Alternatively, you might benefit from the Bulk Send feature which allows you to send using a CSV file with a list of your recipients.
