Not getting DocuSign Template Placeholders Monday

Hi,

when I am using the below action and send document for E Signature then Date and Signhere Placeholder are working fine

when I am using these action these placeholder are not visible while sending document for E signature in the same template

Can you please help me in this, I need to show these placeholders
