The "Recipient Type" set for the Recipient could be set to either "Specify Recipients" or "Update Recipients". These two Recipient Types set in the Template/Powerform would present a screen to add new Recipients or to edit/update existing Recipients.

Ok so I walked through a few of my Powerforms and now I believe I see more clearly your issue...it is not really an error as much as it is another Recipient which the first person to open the Powerform has an option to fill out. I guess I need to ask more about the Use Case for your powerform. Do you not want the person who opens the Powerfrom to be able to see that second Recipient? Is it already pre-filled with a name and email address and you just want to hide that second Recipient?
Correct, the first recipient does not need to see the second recipient. Thank you so much for your help. This is driving me nuts.
Got it....here is how to hide it. Edit your Template, on the Recipients page, go to the Recipient #2, select More, then Advanced Settings and check the two boxes:
Don’t allow senders to edit recipient
Don’t allow senders to delete recipient
Save and close, I always deactivate and reactivate the Powerform just in case so the changes take, then try your test again.
