I have a questionnaire that has a statement and the client has 3 choices to add a checkbox to one of the choices but they need to have the option to choose one out of the three. I need to be able to send the questionnaire which has about 50 questions with the same “checkmark” option to clients.
How do I set this up as a template?

Is it feasible for you to use Radio buttons, which helps recipients select only one option from a group. You can add, remove, or reposition individual buttons. Initially, a Radio field has two options. Use the "+" icon to add more.
