How do I use a custom form to gather address, name, etc., and then have those items automatically plugged into a DocuSign template?

I was not sure if you are using something like SpringCM or another CRM product to try to integrate. Outside of integrations the only option I am aware of to plug in DocuSign field data is the use of Bulk Send. Bulk Send uses a CSV to import information.
