We want to upload the attached PDF file and provide an option to the signer to fill in the information that is requested and then be able to place a check mark in either YES or NO boxes. HELP, can this be done in DocUSign?

If this can be done, would the document need to be uploaded with Adobe PDF fillable fields or rather a blank form with no PDF fillable fields?

Thank you for reaching out here in the DocuSign Community. 

You can accomplish your goal through our eSignature application, and it can be done both with Adobe fields and with a blank PDF with no fields.
