How do I upload the attached document so a signer can complete this form by adding text in the text fields and placing a check mark in the boxes provided, either YES or NO?

Question

We want to upload the attached PDF file and provide an option to the signer to fill in the information that is requested and then be able to place a check mark in either YES or NO boxes. HELP, can this be done in DocUSign?

If this can be done, would the document need to be uploaded with Adobe PDF fillable fields or rather a blank form with no PDF fillable fields?
