Checkboxes don't show up

I have created a template and included checkboxes in the template.  This template is being used in a powerform.  When people complete the powerform, they mark the checkbox as required, but when they download the completed document they cannot see the boxes which have been checked.  The next signer in the powerform also cannot see which boxes have been checked.

I see that the problem pertains to a DocuSign template featuring checkboxes that are designated as required within a Powerform. I noted that after users complete the Powerform, they cannot view the checked boxes in the downloaded document, and subsequent signers are also unable to see which boxes have been selected. I sincerely apologize for the inconvenience and rest assured I'll do my best to help.

 

To assist you more effectively, could you please provide a screenshot comparing the template with the completed downloaded PDF? Please refrain from including any Personal Identifiable Information, as this is a public forum.
