Checkboxes don't show up

Question

I have created a template and included checkboxes in the template.  This template is being used in a powerform.  When people complete the powerform, they mark the checkbox as required, but when they download the completed document they cannot see the boxes which have been checked.  The next signer in the powerform also cannot see which boxes have been checked.

I noted that after users complete the Powerform, they cannot view the checked boxes in the downloaded document, and subsequent signers are also unable to see which boxes have been selected.
