All of my documents go straight to "Completed" instead of to "Action Required" does anyone else have this problem?

the only way I know if there is a new document completed is if I keep track of the number next to "Completed" or by email notification but it never goes to "Action Required". Is there a way to fix this? No one else I've asked has seen this problem before.

I do not know if there is enough information here to suggest what the issue might be, but I suggest reviewing the envelope worfklow and fields. So how many Recipients on the envelope? What are the Recipient Types? How many fields are assigned to each Recipient?

Let's say you have an envelope being sent to 3 Recipients and you select "Receives a Copy" then the envelope after being sent would go to instant "completion" because there are no actions that need to be taken.
