NTREIS Data Input Form

The NTREIS Data Input form doesn’t work for me.  I can’t download the new form, nor can I send it out for a client to be filled out.  When I send through docusign, it takes the client straight to the signature section instead of completing the document.  Am I doing something wrong?  How can I get this form to download?  How can I get this form to send to a client to complete and then sign??
