Conditional Fields with Formula

I have a template with a section that has 16 Yes or No questions. If any of the questions are answered Yes then I need for the next section of information to then show are required test. 

I have used radio buttons for each yes or no question and labeled each radio to mirror the answer of Yes or No. 

Next step I added a test box next to each group of yes or no radio buttons and added the conditional field to each text box if Yes is selected. 

I added my formula for each text box and have them  added to show greater than 0. Then added my conditional field to formula box for required text boxes to show.

However when I click yes the fields do not present at all. 

I am new to Docusign and could really use some help!

Your approach is feasible, but requires many formula fields and a correct configuration of them.

Docusign eSignature formula fields do not support radio buttons, as described here.

Are you able to change the document to have drop downs with the value “Yes” or “No” instead?

If yes, you can set the values to 1 and 0 and have only one formula field to base the conditional logic on.

If you can share a few screenshots we may troubleshoot the issue with your current configuration.
