I forgot my password. I got the code but it asked a security questions that I forgot the answer? How do I log in if this happens?

Are you the only DocuSign Admin on the Account? If not then another DocuSign Admin could reset your password. Otherwise you will need to create a DocuSign case to resolve this issue.
