send a merge document to be signed individually by multiple people

Question

I need to send a document (template so I can insert merge fields) to multiple recipients. Each doc sent would be customized using those merge fields for each recipient. The document requires a single signature from the recipient. I also need to add a custom field as a merge field that will be unique to each of the recipients.
