Hello,
My organization wants to use DocuSign as an internal review and approval process for documents. I don’t see an option to add a recipient as a Reviewer only. Is there an Action button for Review?

Welcome to the DocuSign Community!
In DocuSign eSignature there is not an specific option for review only but the one thing you can do, is to while creating the envelop you can set the recipient as “Need to View”.
Not sure will fulfill your requirement but hope this helps.
