Would it be possible for you to provide the envelope ID? to review the information at the backend?

How do I find an Envelope ID as a recipient/signer?

As a recipient/signer, there are several ways to find the Envelope ID. If you have an active Docusign Account, you can find it by following the provided steps. If you only have access to an email notification, you can copy the link address or use the Access Documents security code. Alternatively, if you have downloaded the documents previously, you can locate the Envelope ID in the upper left-hand corner of each document. If none of these methods work, it is recommended to reach out to the sender of the envelope.

Note: Some advanced features and options are supported only in specific Docusign plans. Your account plan might not support some options discussed in this help topic. Contact your account administrator for more information about which options are available for your account. This signature looks quite weird, it’s been altered. I would contact the sender of the envelope and ask them clarification and potentially send you a new envelope.
