How to customize a template

New user/administrator.  Have uploaded a .docx gile to use as a template.  Can’t figure out how to insert custom fields into that template.  Want to insert dates, fees, signature fields.  Will then send template to recipients one at a time in different envelopes.  Each envelope will be an agreement with different dates, fees, recipients, etc.
